Th/To antigen (or RNase P Nucleolar Protein Complex, Rpp 25 Subunit) is composed of two RNA processing enzymes. Anti-TH/To autoantibodies produce a bright nucleolar or nucleolar and speckled pattern on ANA testing.
Anti-Th/To is associated with limited cutaneous Systemic Sclerosis (lcSSc), with excellent prognosis. Although patients seem to have frequent pericarditis and Interstitial Lung Disease (ILD), impairment of pulmonary function appears mild.
Although the autoantibody is predominant in Systemic Sclerosis (SSc), it also occurs in other diseases like Systemic Lupus Erythematosus (SLE) or Polymiositis (PM) where it may indicate the future development of an overlap syndrome with bowel, pulmonary or renal involvement.
